Job Summary:
The Division of Academic Specialists in Obstetrics and Gynecology at the University of Colorado, School of Medicine, has 1 Senior Instructor Faculty Position available for a LCSW. The position is a 0.9 FTE. We invite candidates with at least one (1) year of LCSW experience to become a part of our team at the University of Colorado Anschutz.
Job Responsibilities:
The Pregnancy Medical Home program in the OB-GYN UCHealth practice is a multidisciplinary team within the department of Ob/GYN providing longitudinal medical and social support to pregnant and postpartum moms. This program targets delivery of prenatal care and transition of care after delivery to interpregnancy care for the Medicaid patient population at UCHealth via creation of a Pregnancy Medical Home. This unique care model is expected to both improve the quality of patient care delivered and maternal and newborn health outcomes. In addition to the social worker, the programemploys a full-time nutritionist, two full-time nurses., a researcher, two nurse-midwives, and an OB-Gyn.
The Social Worker will work within a team setting to assess patient needs at entry into the program. They will provide screening interviews to identify at-risk social and behavioral patients entering the clinic. They will provide education and counseling addressing behavioral and social needs of patients. They will coordinate referrals to resources within the clinic, campus, and community to further address medical, mental health, behavioral health, and social needs of the patient. They will provide education to patients. They will provide outreach and education to community resources about the program. When appropriate, the position will triage to interdepartmental and community resources to provide additional multidisciplinary care.
When identifying at-risk patients this position must be able to make critical decision regarding the patients’ mental health needs and social work assistance. Ability to work within a team setting that includes clinical psychologists and psychiatrists is critical.
Position must be able to identify and create a list of community resources for individuals needing long-term care. Position must be able to assess the patient’s needs and provide them with the proper clinic and community referral.
