Greater Los Angeles Healthcare System (GLVAHCS) is seeking 1 highly skilled experienced Registered Nurse (RN) for our Community Living Center Minimum Data Set Unit. Located in Los Angeles, CA.The Nurse II Community Living Center (CLC) Minimum Data Set (MDS) Registered Nurse (RN) is responsible for providing competent, evidence-based care to assigned residents and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting. The nurse will assist in directing the provision of nursingeducation, orientation, competencies and providing quality improvement and outcomes utilization consultation. Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. Responsible for coding the sections of the Resident Assessment Instrument (RAI)/MDS Assessment and the Care Area Assessments (CAAs) that are assigned to nursing, adhering to current Centers for Medicare and Medicaid Services (CMS) and Veterans Health Administration (VHA) timeframes of assessment completion. The MDS RN in such a designated position is only responsible for the completion of assigned nursing sections in the RAI/MDS Assessment. Participating in the CLC Interdisciplinary Team care plan meetings and reviewing, communicating, and monitoring the residents' care plan. Collaborating with the CLC Resident Assessment Coordinator (RAC) to educate CLC staff on the completion of the RAI/MDS process; this includes but is not limited to documentationrequirements for accurate MDS data entry, educational sessions pertaining to RAI/MDS software and program updates as needed. Supporting quality improvement activities based on CLC quality management data. Participating in ongoing survey readiness activities in the CLC. Documenting and taking corrective action when notified by the CLC management team, charge nurse, or designee of any discrepancies in coding or documentation of RAI/MDS assessments and CAAs reported during the interrater reliability review. Provides peers with informal constructive feedback for improvement. Fosters a safe and supportive environment conducive to the professional development of healthcare professionals. Supports colleagues through knowledge sharing to provide safe, quality nursing care. Questions clinical practices for the purpose of providing evidence-based care.Work Schedule: Monday - Friday, 07:30 am to 04:30 pm. May include Weekends and Holidays.
